Simply complete this portion of the statement and return it in the envelope provided, or call Patient Accounts at (219)326-2310 and an Inquiry Representative will take your information and mail you receipt.
Each time you come to our facility, your visit will be assigned an account number. This is the number you will reference when inquiring about your account. If you frequent our facility, you will have multiple account numbers. To eliminate confusion, when you receive statements, you should group them by account number with the most recent statement on top.
