Membership

Foundation members support the Annual Fund. The funds are allocated by the Board of Directors for the purpose of providing hospital equipment and health science scholarships, and assist the Community Health Center, La Porte County Dental Clinic, and other health-related programs.

Members have privileges for board elections and other pertinent issues.

Benefactor’s Club - $5,000 and above
21st Century Club - $1,000 to 4,999
Chairman’s Club - $500 to 999
Samaritan Membership - $250 to 499
Ambassador Membership - $100 to 249
Contributor Membership - $50 to 99
Hospital Family - $25 to 49
Friends - $10 to 24

Corporate Benefactor’s Club - $5,000 and above
Corporate 21st Century Club - $1,000 to 4,999
Corporate Chairman’s Club - $500 to 999
Corporate Samaritan Membership - $250 to 499
Corporate Ambassador Membership - $100 to 249
Corporate Contributor Membership - $50 to 99
