Heart Failure Program

The Heart Failure Program was developed to assist heart failure patients and their families to understand a heart failure diagnosis. This free program provides education and promotes patient and family involvement thereby, decreasing future hospital visits and increasing independence and quality of life.

Rebecca HerroldLRHS employs a Heart Failure Nurse Coordinator who specializes in heart failure. If you are diagnosed with heart failure during an inpatient stay, the nurse coordinator will meet with you and your family to discuss medications and the importance of daily weigh-ins. The nurse coordinator will help you and family members identify signs and symptoms of heart failure, and when to call your physician, 911 or come to the emergency department. After discharge, the nurse coordinator will call you to review medications, daily weights and ask if you are experiencing increased swelling or shortness of breath and answer any questions you may have.

Participation in the Heart Failure Program can minimize complications during the transition from the hospital to home and extends the opportunity to learn and self-manage a heart failure condition. Participation in the program is voluntary and does not require a physician referral.
