Hospice for Heart Disease

The mission of the VNA Hospice program of La Porte Regional Health System is to offer compassionate care to individuals who are at the terminal stage of life. Hospice assists with physical, emotional and spiritual needs, while providing care with an emphasis on specific disease symptom management. VNA Hospice allows individuals to live every moment with dignity and share quality time with family and friends.

VNA Hospice provides compassionate support in the comfort of one’s own home, while helping families cope with the stress of caring for a terminally ill loved one. Services are provided by highly trained, experienced healthcare and spiritual care professionals as well as volunteers who teach and prepare families how to deal with loss and grief.

Claiming more lives than all cancers combined, heart disease continues to be the No. 1 killer of Americans. Studies suggest that people with end-stage heart disease suffer more than people with advanced cancer therefore; hospice isn’t just for those who have cancer. Individuals living with heart disease are often faced with frequent hospitalizations and expensive therapies when they should be enjoying life surrounded by family and friends in the comfort of their own home.

Individuals with end-stage heart disease may qualify for VNA Hospice if they experience the following symptoms:
      • Shortness of breath at rest or lying down
      • Shortness of breath doing normal, daily activities
      • Swelling in hand, legs or feet
      • Chest pain, dizziness or increased weakness
      • Chest pain that results in multiple visits to the hospital or emergency room
      • Frequent coughing
      • Medication changes

Services include:
      • Coordinated medical care by the primary physician and the Hospice Medical          Director
      • Symptom management
      • Skilled nursing care
      • Home health aides
      • Social services and counseling
      • Holistic care for the whole family
      • 24 hour on-call services
      • Spiritual counseling
      • Physical, occupational and speech therapy
      • Nutritional services
      • Pharmaceuticals
      • Volunteer services
      • Bereavement counseling for 13 months following death

Making a referral to a hospice program can be difficult due to advances in technology and the fact that individuals with end-stage heart disease are not necessarily bedridden or homebound. Technology provides longer life spans and higher survival rates. The decision to refer to a hospice program is based on the disease’s natural course, treatment options and an expectant life span of less than six months.
