The Continuing Care Center is a hospital-based, skilled and extended care facility offering a wide range of services to short and long-term patients. Our facility provides additional care for patients recovering from surgery, illness, or injuries that require additional observation and/or treatment. It is ideal for individuals who require physical and occupational therapy to better prepare them to return home and to work. Long-term care is available for individuals living alone or whose physical and medical conditions require care beyond the ability of family members.
To assure progressive patient recovery, an individualized treatment plan is developed upon admission. The treatment plan is based on an individual's physical and emotional needs, anticipated progress and other appropriate assessments. The plan identifies recovery goals, needed services, physical abilities, and discharge planning.
The Continuing Care Center accepts commercial insurance, Medicare and Medicaid. It features:
